Bonggo already complied all requirements in her subject stated above; 7. That I have signed the complied INC form of Ms. Elizabeth N. Bonggo due to a valid reasons. That this Affidavit is executed by me for the purpose of attesting the true facts above stated and to request Commission on Higher Education Region 10 to accept the INC grade completion of Ms. Elizabeth N.
