In 1973, after her daughter was diagnosed with an incurable neuromuscular disease, Constance Webb sought to use the healing properties of spiders that found in the Amazon Rainforest of Peru considering the myths of these spiders granting its bite victim grandiose spider-like powers and abilities. She was accompanied by Ezekiel Sims, who would shoot her down to take the spider she eventually found for himself and his own personal gain. Before she could die, she was found by a tribe of spider-people known as "Las Arañas". Their leader, Santiago, provided a spider to bite her mother and preserve her daughter's life in trade of that of Constance's.

In 2003, 30 years succeeding Constance's death, New York City Fire Department paramedics Cassie Webb and Ben Parker arrive at the hospital saving the life of a woman on the way. After those events, Parker tried to invite Cassie to his nephew's baby shower but she denied since she isn't as good socializing. Cassie would later retrieve a box containing her mother's belongings under her bed at her apartment after being reminded of her family by a drawing that the rescued woman's son gave her. She flips through the pages of her mother's notebook, in which she had written down her spider-related research, before closing the box.

Disallowed to drive an ambulance vehicle by O'Neil, learning of an upcoming car crash, Cassie lets it happen and tries her best to revive him, to no avail. Later that day, Sims, after having a spider crawl down his hand and putting it away, is shown by his research assistant Amaria what the women in his visions from the future may look like, aging them down to their current ages. Surprised, Amaria contemplates Sims' plans to harm teenagers, which Sims does not plan to back down from. He promises Amaria a huge fortune if she finds them.

Following a visit by the doctor, Cassie rests in her apartment while her precognition takes effect once again; After seeing a pigeon die by hitting her window, Cassie saves the bird by opening the window, allowing it to fly inside and back outside. She later visits the Grand Central Terminal in order to travel to Poughkeepsie, New York to attend O'Neil's funeral while Sims nears the location, told by Sims that the teenagers, Julia Cornwall, Anya Corazón and Mattie Franklin, are coincidentally all at the same place at the same time. Said girls enter the train as Cassie knows they will be attacked by a costumed man. She escorts the girls out of the train, seeing Sims still inside as the train passes by. Corazón asking what the emergency is and Cornwall assuming Cassie is abducting them, Cassie points to the costumed man crawling towards the women. Cassie and the Boo Crew make their escape in a taxi while the man apprehends police forces. After hearing how she is wanted by the police for possible child abduction, Cassie drops the Boo Crew off in the woods of Church Hill, New Jersey, where she learns how she has met them before; having saved Cornwall's stepmother, living in the same building as Corazón and being flipped off once by Franklin. She leaves the girls, promising she'll be back within three hours and entrusting Cornwall with the rest's protection, to investigate on her precognitive abilities. She learns of an ancient tale of Las Arañas and their spider-like abilities and where she recognized Sims from, him being her mother's partner and the man who was granted such powers, targeting the girls.

The Boo Crew become hungry and decide to enter a nearby diner, Amaria tracing a phone call from a man recognizing the three reportedly missing girls. Cassie returns to see the girls gone, finding them at the diner being attacked by Sims once again, Franklin dying from his poisonous touch and Cassie being stabbed. Cassie's vision ends, prompting Cassie to leave earlier and run Sims over with the taxi. Cassie is poisoned as she drives the Boo Crew to a motel while Sims is told Cassie's last name: "Webb," much to his surprise. Cassie snidely remarks that their parents should deal with the situation, but she is told that there is no one to look after them, which Cassie can relate to. She vows to protect them and puts them to sleep, using her precognition to return to the diner. Her abilities help her talk with Sims, who explains his motives to Cassie and urges Cassie to tell them where the girls are, proclaiming that they are useless to her anyway. She awakens in the morning, waking the girls up to teach them CPR in case Sims' poisonous touch causes heart failure to anyone. Telling them of Sims and his possible connection to Las Arañas, Cassie leaves the girls to Parker while she researches her origins in Peru. She meets Santiago, the tribal chief of Las Arañas that casts her body into astral projection so she can learn why her mother was in the Amazon rainforest when she was researching spiders right before she died: to treat Cassie's hereditary disease. She witnesses the events that occurred through multiple visions, comforting a projection of her mother. Afterwards, Cassie is told that the visions were partly her doing and that she can master the powers her mind has to offer when taking the responsibility, as great power will come.

A week later, Parker's sister Mary's water breaks, leading to Ben driving her to the hospital with the kids laying low inside of the car. Even so, Amaria spots Franklin's face and alerts Sims, who tells her to turn every traffic light in the city green as a returning Cassie is alerted of an upcoming attack by her precognition. From a building, Sims leaps down on the still car in which the Parkers and the Boo Crew are trapped, but is hit by Cassie and her stolen ambulance. She drives the girls away with said ambulance with Sims on her tail. The Boo Crew thwarts him. The ambulance breaks down, allowing for Cassie to come up with a plan. Luring Sims into the building near the scene of O'Neil's death, Sims is distracted by the abundance of exploding firecrackers, one launching into a wall and breaching a hole that Cassie and the Boo Crew run through. An officer piloting a helicopter she called to the building instruct the women to go to the south side of the building's ceiling, Cassie protecting the girls from the firecrackers using her visions along the way. Told to go higher on the building, Cassie continues to protect the girls as Sims arrives, destroying the helicopter with a deflected firecracker. Sims nearly accomplishing his mission, Cassie uses the Web of Life to bring the girls to safety from their impending doom. On the building's catwalk structure, Cassie tells Sims that she was the deciding factor in his future, not the girls, before he is crushed under the building's debris. Cassie falls as well, but into the nearby ocean, where a firecracker passes her eyes, blinding her. She is rescued by the Boo Crew.

Madame Web and the Boo Crew

While Ben accompanies Mary's newborn son, Peter Parker, in the hospital, Cassie is joined by the girls, later inviting them in her new apartment once fully recovered while paraplegic. She is asked about her eyesight, to which she proclaims that she can see even better than she has before, seeing a future where the girls operating as Spider-Women.
