Our long-term care facility is seeking a dedicated and experienced Social Services Coordinator. As a Social Services Coordinator, provide medically related social services that assist the residents to attain or maintain the highest practicable physical, mental, and social well-being. The Social Services Coordinator will manage and oversee the specialists and vendors the visit the facility.

Social Services Coordinator Key Duties

• Manage the facility in the absence of the Social Services Director.
• Conducts pre-admission screening of individuals seeking admission to the facility, including persons with mental illness according to facility admission criteria.
• Completes and updates social services assessments for residents.
• Function as Care Plan Coordinator as assigned.
• Works cooperatively with members of the interdisciplinary team to develop, implement and evaluate plans of care to protect and promote residents’ rights.
• Records progress notes in the clinical record. Observations of resident’s behavior and interventions
• Perform other job-related duties as they become required.
  
  
  

Social Services Coordinator Qualifications

• Bachelor’s degree required.
• Minimum of 2 years’ experience in long-term care or a related setting.
• Knowledge of local, state, and federal regulations and Dept. of Health Survey process.
• Ability to work effectively as part of a multidisciplinary team.
