Cover Story (view full-size image): Gluten-related disorders include celiac disease, wheat allergy and non-celiac wheat sensitivity, which have in common the dietary exposure to gluten and other wheat derivatives as an etiologic factor. Of notice, each present a unique pathophysiology: celiac disease is mainly a T-cell mediated condition; wheat allergy is characterized by a rapid IgE-dependent or -independent immune response and the basis of non-celiac wheat sensitivity is an innate immune response. Impairment in the epithelial barrier function is a common feature in gluten-related disorders. However, whether it is a primary condition within the pathophysiology of these diseases is still a matter of debate. The paper gives an overview of the reported barrier defects in gluten-related disorders and supports the hypothesis of a primary involvement of the epithelial barrier function in the development of those conditions.
