The revenue to a #hospital system is usually described as at least 5-10 times the cost of the physician. Physicians become a loss leader - though the systems will cry, "We're losing money on you." Additionally, the outpatient office billing gets lost having been absorbed by the system billers, who pay more attention to big bills, and not the visits, office testing and revenue from office - thus they get undercollected. Additionally, the outpatient office becomes part of the system's HR problem, and the lowest paid employees. Receptionists, office staff and clinical help become an entry pathway to better paying hospital jobs- thus turnover is high - a death knell to an office practice.

Insurance doesn’t take care of anyone !! And the reason it doesn’t make money from the primary care standpoint is because of all the nonsense and hands in the pot! Take away the PBM‘s, the pharmacist tech who is supposed to be my peer for the pre-authorizations, the administrators who make 80% of our profits, and the ridiculous metrics. Primary care was made for prevention . We can’t even do that with nonsense 10 minute visits . Keep insurance for catastrophes, not what we do . Do you use your car insurance for maintenance or your homeowners insurance for updated appliances ?!
