Confined space entry is a critical procedure that should never be underestimated. Only trained and qualified persons should enter confined spaces. Remember to review and sign all LOTO and Confined Space Permit Procedures before entering such spaces. It is crucial for facilities to thoroughly evaluate what spaces are considered confined spaces to ensure personnel safety. Have you ever wondered how these things happen with all the advanced engineering, required permits, planning, implementation, quality control/assurance, safety regulations, inspections and more? Well, in short something in the forementioned process(es) failed. I'll address one part of these and that is "inspections." Recently I informed a perspective client that since they did not have any Weld Procedures we could not inspect his project to any Code. His response was, "its taken care of." Not saying this is what happened in Boise, but too many times we see parts of the above processes ignored to save money or increase profit, which is gambling and could lead to a catastrophic event as below. On the welding inspection end, four components I believe you should ALWAYS start with and have a Certified Welding Inspector (CWI) review and approve before your project is awarded to a subcontrator: (1) Welding Procedure(s) (2) Welder Qualification(s) (3) Continuity Records and (4) Material.
