Where will money flow into kidney care in 2024? A few weeks ago I shared a glimpse at the tech-enabled kidney care landscape. Today I thought a more detailed view of growth and funding might be in order, especially as predictions season looms near. First, here's what you're looking at: ��� (4) segments: VBC, RPM, Diagnostics, Devices → 8-10 companies each (10 ppl min.) → 1-year avg headcount growth, by segment → average funding / employee, by segment Now, a few takeaways: (a) GROWTH: Value-based care remains the fastest growing segment measured by 1-year headcount growth at ~23%. That's pretty significant considering it's also the area of kidney care with the highest average headcount (345 employees) and nearly $2B raised in funding. Remote monitoring (RPM) and Diagnostics (AI/ML) both came in around 15%. The laggard is the device segment, which came in at ~6%, or about 4x slower growth than VBC. 👑 VBC: Panoramic Health 👑 RPM: Kalium Health 👑 AI/ML: DeLorean Artificial Intelligence 👑 Devices: Diality (b) CAPITAL EFFICIENCY: It might surprise you then that value-based care comes in at the lower end of funding per employee. Diagnostics ($361K) and value-based care ($388K) came in well below medical devices, which shows $615K per employee. (c) CAPITAL REQUIREMENTS: Keen eyes might have noticed the dotted line running left-to-right halfway down the table. This is a rough split between services and devices. The reason for this is the interplanetary distance between the amount of capital required to stand up a care delivery company vs. bringing a novel medical device to market. Many of you will know this firsthand. Bill Hunter wrote one of the best posts I've seen on how the bar has changed for medtech exits since 1998 (link below). Notes: • These groupings are based on primary use cases and value propositions to the best of my knowledge. There are companies with device & hardware components in the diagnostics and RPM categories that likely skew the funding and efficiency metrics (e.g. Renalytix, CloudCath, Proton Intelligence, Alio, Biofourmis) compared to their software-only or software-enabled counterparts. • This is non-exhaustive. I'm focused on kidney care applications. There is a growing consensus of reserch published around the postive impact of First Contact Practitioner Physiotherapists 🙌🏽 The recent GP DES highlights the importance of patient feeback, audit, clinical outcomes and services improvements and with upto a third of GP appointments being for an MSK issue, analysing the impact is essential 👍 This great 2024 paper and well worth a read if you work in #primaycare. The study had 426 participants and looked at several different factors and impacts. Please do take the time to read it. Here is my interpretation and brief summary points: Key Benefits of First Contact Physiotherapy Practitioners (FCPP) in General Practice: ✅ Expert MSKD Skills Access: FCPPs provide specialised expertise in managing musculoskeletal disorders (MSKDs), addressing up to a third of GP workload related to these conditions ⚖️. ✅ Safe and Effective Alternative: FCPP-led consultations are proven to be safe, clinically effective, and cost-beneficial compared to traditional GP-led care 🤝. ✅ Reduced Medication Usage: FCPP consultations result in significantly lower prescription rates, especially for opioids 💊, contributing to better patient outcomes and decreased reliance on medication 🙌🏽. ✅ Quicker Patient Improvement: Patients consulting FCPPs experience faster improvement, reducing the duration of illness and enhancing overall quality of life 💪. ✅ Cost Savings: Embedding FCPPs as a standard model in general practice leads to substantial cost savings for both patients and the healthcare system, making it a financially viable option 💷😊. ✅ Reduced GP Consultations: With FCPPs available, there is a reduction in the number of MSKD consultations with GPs, alleviating the workload pressure on primary care physicians 😌. It was a pleasure to assist to this second event organised by Vithas and Dr. Carlos Tornero Tornero • The demografics of the Valencian Community 🇪🇸 regarding chronic pain incidence and management are concerning. 12% of the population affected by chronic pain is forced to permanently leave the work place because of it. Cervical and cranial pain are the 🥉 (7.5%) in order of appearance among patients after the lumbar and spinal ones. •In EEG imaging 🧠 the cerebral appearance of chronic pain is shown to migrate from the frontal-somatic areas to the lymbic-emotional ones, highlighting the strong relevance of patient feelings and resilience in its management. • The “Sine qua non” of most chronic pain is related to bad lifestyle and unhealthy habits of patients, which in the long run ⏳lead to its appearance. • Overall invasive treatment 💉cannot be a permanent solution to chronic pain, though they can be used as a helpful tool providing a reduced pain period in which the patient, with the help of a specialised professional (physiotherapist), can introduce new and better lifestyle habits aiming at an overall health improvement. • In conclusion a: - Multidisciplinary approach 🩺🔬💉 - Aiming at the reduction or resolution of pain - Trough the education of the patient 🧑🏻🏫 Should be routinely implemented at any practice and specialty level along the patients recovery journey.

Very good article highlighting a growing problem in our communities. As vascular surgeons we are often called on to help these patients who have had numerous unnecessary procedures and now have serious complications. There are very few patients who receive LONG TERM benefit from intervention for simple claudication symptoms. Most would do better with medical therapy, a monitored walking program, and lifestyle modification. Unfortunately this is almost never presented this way to the patient. They are promised a simple treatment and when there are complications, then they are sent to us to fix the problems. Vascular surgery should be a game of defense , not offense. As vascular surgeons we have many tools in our toolbox, but should only be usuing them when necessary to clearly benefit the patient.

What happens when several of the most powerful and entrenched organizations come together to protect their own self-interests? Patients and providers suffer. Case in point: AHIP joined the American Hospital Association (AHA), American Medical Association (AMA) and Blue Cross Blue Shield Association (bcbsa) in opposing the prior authorization reformation efforts from CMS in a notice of proposed rulemaking, laying out their arguments in a letter to CMS. At Zula Health, we see both the value and frustration of prior authorization. With large ticket items, such as cardiac surgery or new treatments, it makes sense to have a process in place to ensure that the necessary evaluations and therapies have been taken into account and shown that the recommended course of treatment is the right choice. We can even see the value and even need to pre-evaluate options for self-insured groups working through a third-party administrator. These approaches work because they are focused on two things: patient health and proper delivery of care. Where we believe that prior authorization fails is when large health systems leverage the process to make it more difficult for patients to readily access care covered by their health benefits. Specialty care is a perfect example of this: many large carriers use prior authorization to either limit or create a barrier between patients and alternative health, such as chiropractors and acupuncture. The general belief I have found through many conversations with payers is that there is a fear of abuse of these benefits if they are not closely monitored. Given that most people access these providers (as part of the benefits they pay for) for either chronic or acute pain management, one would think that payers would rather have members access a DC for a few hundred dollars than visit the emergency department for several thousands. Or, even to avoid opioids. But maybe the #PBMs might argue against that. We believe prior authorization can be replaced in specialty networks with ‘in-sourcing’ where payers have a more direct and trust-based relationship with a high-quality provider network with whom they both credential and contract.
