Today, FDA published a guidance with the title: Considerations for the Conduct of Clinical Trials of Medical Products During Major Disruptions Due to Disasters and Public Health Emergencies. FDA led the global community in rapidly responding to disruptions resulting from the COVID pandemic including by publishing a guidance on managing disruptions to clinical trials. The concepts in the guidance published today are based on the details in the guidance that was published during the COVID-19 public health emergency addressing disruptions to trials caused by the pandemic. The new guidance addresses major disruptions due to disasters and public health emergencies in general and is not limited to COVID. Why this is important? This guidance highlights that lessons from COVID are not forgotten and in fact, they are being considered as appropriate for major disruptions to clinical trials regardless of cause. This effort is informed by the clinical trial community's input and it highlights that we are collectively learning and adapting. Efforts are still ongoing to determine how to make such guidances even more useful to the clinical trial community. As we prepare to culminate our Combat Lifesaver training this week, I’m building a comprehensive whole blood transfusion session for both medical and non-medical personnel. I’ve found invaluable resources from the Next Generation Combat Medic website, complementing the wealth of knowledge from Deployed Medicine. On deployment, this is a crucial task for all members of the 4th Security Force Assistance Brigade in our small team layout. In 2014, the Joint Trauma System (JTS) DoD Center of Excellence for Trauma championed whole blood as the optimal resuscitation fluid. The 2018 update from the Committee of Tactical Combat Casualty Care (CoTCCC), noted the preferred resuscitation fluids for casualties experiencing hemorrhagic shock are ranked from most to least favored: 1. Whole blood 2. Plasma, red blood cells (RBCs), and platelets in a 1:1:1 ratio 3. Plasma and RBCs in a 1:1 ratio 4. Plasma or RBCs alone 5. Hextend 6. Crystalloid (Lactated Ringer’s or Plasmalyte-A) Normal Saline is not on this list. The National Center for Rehabilitative Auditory Research (NCRAR) was established in 1997 at the Veterans Affairs (VA) Portland Health Care System (VAPORHCS) in Portland, Oregon. The NCRAR is one of 13 research centers supported by the VA Rehabilitation Research & Development (RR&D) Service. Each of the RR&D centers focuses on a specific health condition that is of relevance to United States military Veterans. The NCRAR is the only RR&D center that focuses on auditory and vestibular research. Whereas other researchers are focusing on finding a cure for tinnitus (permanent elimination or reduction of the tinnitus sensation), the NCRAR’s focus has been on developing and testing different methods of tinnitus clinical management with the overall purpose of validating clinical methodologies for routine application.
