Overview:
Psychiatric Rehabilitation Services (PRP) are therapeutic supportive services provided in the community by a well trained Client Care Coordinator for youth and adults who are active in mental health treatment/therapy. The PRP goal is to enable our clients to be successful and satisfied in the environment of their choice by working together to develop useful goals.
Duties:
- Meet with clients regularly and consistently to work on client goals (6 visits a month for adults and 3 a month for minors)
- Conduct in home or community based visits to provide individualized coaching and skills training as needed
- Link clients to government and community resources and assist in obtaining those resources
- Ensure client maintenance of physical and mental health, and well being
- Provide and/or coordinate transportation to and from scheduled appointments if needed
- Complete all documentation including in a timely manner
- Report regularly to direct supervisor
- Ensure compliance with all agencies policies and procedures and State regulations
Qualifications:
- BA degree in human services, art, or social services with at least 1 year working with clients. Or a high school diploma with some coursework in social services or human services
- Ability to critically think, analyze, evaluate and develop solutions for clients needs
- Excellent interpersonal skills and ability to build relationships with individuals of all ages, ethnicity, education, and work experience
- Valid Drivers License
- Knowledge of behavioral health principles and interventions
