Position Function Summary

Under the general direction of the Director of School-Based Health (DSBH), the School-Based Health Program Manager (SBH-PM) is a Registered Nurse who oversees and manages a variety of patient care services that include and extend beyond routine clinical visits. The SBH-PM is tasked with coordinating the scheduling of healthcare services at off-site locations, including ensuring adequate staffing, equipment and supplies, event site/spaces, and when appropriate, patient scheduling templates. The SBH locations include Department of Education School-Based Care Sites, Mobile Health Units, project-based housing units, mass vaccine/testing events, and may include other temporary locations as needed. Additionally, the SBH-PM oversees multidisciplinary group visits and other scheduled outreach events. The SBH-PM is responsible for the supervision and leadership of all School-Based Health staff, including those permanently assigned and those temporarily assigned for specific events.

Ii. Essential Duties And Responsibilities

This position may have various work assignments within HICHC. This description is intended to be generic and does not detail all duties of the job assignment. Various duties, responsibilities and accountabilities may be assigned to an incumbent in this position depending on clinic needs, and may include but not be limited to the following:

Supervisory Duties

  • Works with the Director of School Based Health, Senior Director of Clinical Operations (SDCO), Medical Director, Senior Director of Nursing (SDON), and Director of Health Equity to ensure there is an integrated healthcare delivery system that is efficient, effective, and timely with appropriate response to patient and community needs in a variety of settings.
  • Plans and oversees health service delivery by School Based Health team, makes staffing assignments and supervises and evaluates the quality of care delivered.
  • Coordinates and schedules events with a variety of internal and external partners to meet the community's needs and support the HICHC clinical teams.
  • In coordination with Quality Improvement (QI) team provides leadership for continuous quality improvement efforts. Implements and/or updates processes for data collection, tracking and documenting activities.
  • Provides leadership to ensure the highest standards of excellence in customer service by anticipating and responding to needs of internal and external partners; proactively work to improve the quality of customer service.
  • Provides support for clinical activities performed by the School Based Health team by acknowledging, adhering to, and implementing organizational, nursing, and medical policies and standard operating procedures.
  • Ensures timely and accurate documentation as required in medical software applications for scheduling, charting, and billing for HICHC and additional documentation in external databases for program management and grant funding.
  • Actively seeks continuing education and training tools to support best practices for program and staff development and shares information through staff in-service training and individual performance improvement training. Conducts and documents regular team meetings.
  • Manages inventory to ensure that necessary supplies are ordered and available, are properly stored and maintained, while seeking ways to reduce waste of resources and supplies.
  • Maintains compliance with State licensure by ensuring Quality Assurance protocols are adhered to in all patient care areas.
  • Ensures regulatory compliance of HIPAA by monitoring the workplace environment, on- and off-site staff activities, and patient care services provided. Identify areas where improvement or training is needed.
  • Handles patient complaints and/or questions related to assigned program areas and addresses appropriately or forwards to Director of School Based Health expeditiously.
  • Collects and compiles data and submits all reports required for grant funding utilized to maintain the School Based Health Department.
  • Develops, reviews and updates policies, standard operating procedures and documentation tools as required to perform services in a variety of settings in collaboration with the DSBH and SDCO, Medical Director, SDON, and QI team.
  • Attends budget and grant-related meetings, completes grant reports as required and works with leadership and finance department to appropriately utilize grant funds and resources.
  • Completes other projects and duties as assigned by DSBH, SDCO, COO, and CEO.

Nursing Duties

  • Follows Nursing Practice Standards to evaluate patient needs and provide nursing services within designated scope of practice.
  • Documents all patient care provided including assessments, interventions, preventative care and education, follow up and recommendations.
  • Communicates with a medical provider about patient needs, nursing assessments, interventions and patient education promptly.
  • Acts as a patient advocate. Coordinates interdisciplinary services and referrals to HICHC and other community resources as indicated.
  • Educates patients by disseminating patient information and literature relating to health education programs including disease prevention, family planning, pre-natal care, tobacco cessation and health promotion for all age categories.
  • Performs direct patient care, utilizing established procedures, policies, and standards as evidenced by client/staff feedback and physician observation.

oAssesses, collects, and documents the patient’s current health status and medical history. Makes accurate initial and ongoing nursing assessments and responds appropriately. This may occur in person, at outreach events, scheduled appointments, and via telephone calls.

oDemonstrated ability and appropriate technical skills when administering medications, IV’s, medical/nursing treatments, and nursing procedures in accordance with medical provider's orders, standing operating procedures and HICHC policies. Assesses and documents response to treatment and compliance with clinical regimens.

oDemonstrates necessary skills and knowledge to provide care for patients of all ages according to policy and procedure and primary care nursing competencies.

oDemonstrates ability to handle emergencies in a prompt, precise, professional manner per nursing training and HICHC protocol.

oPerforms patient care to provide comfort and well-being to the patient, acknowledging physiological and psychological needs.

III. POSITION SPECIFICATIONS

Requirements Of Position

Education And Experience

  • Bachelor's degree in nursing required.
  • 1 year of supervisory nursing experience (charge nurse experience is applicable) required; 3 years progressive experience in a community health center and working within a clinical health care setting preferred.
  • In lieu of a bachelor’s degree, the incumbent must have an associate degree in nursing and possess sufficient prior work experience in progressively responsible positions during which the skills and abilities to perform all the essential functions have been amply demonstrated at a consistently competent level.

Licenses and Certifications

  • Current Nursing license for the State of Hawaiʻi is required, and documentation of education, experience and licensure is required and must be maintained; incumbent must be insurable under the policies and requirements of HICHC.
  • Current BLS certification is required and must be maintained.

Knowledge, Skills And Abilities

The School Based Health Programs Manager must be familiar with the community health care field and must have a sound understanding of and ability to apply leadership and management principles and methods. Additional knowledge and skills important to this position include but are not limited to:

  • Demonstrates knowledge of the principles of growth and development over the life span and incorporates them in program development, patient care and health education.
  • This position requires advanced knowledge of patient scheduling, intake procedures, laboratory, pharmacy, and billing protocols.
  • Interpersonal relations and customer service skills; facilitating interdepartmental operations.
  • Knowledge of the applicable administrative standards, rules and regulations affecting provision of healthcare services provided in off-site events.
  • Good organizational and self-management skills, including time management, document processing and reporting, and management of a broad scope of responsibilities.
  • Must be able to work on multiple tasks within established deadlines.
  • Must be able to work independently under the general direction of a provider and follow instructions for work completion.
  • Working knowledge of computer systems including but not limited to word processing, spreadsheet, database, and electronic communication software applications.

Language Skills

  • Ability to read and interpret documents such as patient records, operating and maintenance instructions, procedure manuals, etc.
  • Ability to write at an above average level of competence, including internal reports and memoranda.
  • Ability to communicate with diverse groups of people to include staff and providers and patients.
  • Ability to communicate effectively with patients and their families to make their visit a pleasant experience.
  • Sensitivity to the multicultural nature of the service area population and may be required to communicate in another language.

Physical Requirements

  • Prolonged periods of sitting at a desk and working on a computer
  • Prolonged periods of standing, bending and reaching
  • Must be able to lift up to 20 pounds at times
