The only FQHC in Brevard County employs approximately 65 physicians and APPs.
We work with existing providers in our community to coordinate service to our patients.
Following a patient-centered medical home model, with ID department within the confines of primary QUALIFICATIONS:
Lead Physician for Infectious Disease- working alongside APRN and per diem Physician in coordinating care for patients including: HIV, Hepatitis C, Chronic Hepatitis B, STD, and other outpatient acute and chronic diseases
Board Certification Required -Must see adult patients only
Active DEA and BLS
Spanish is a plus
Proficiency with EMR
Must understand 340B, Ryan White, and coordination of HIV services
