Nursing Clinical Preceptor - HYBRID

  • Monitor the new hire's work processes and communicate progress to appointed supervisor; case management skills; competence with computer softwares; nurse/patient interactions; clinical competencies, communication with other healthcare providers and within IDT; documentation standards.

  • Carries a modified panel of patients and assists with managing panels vacated by RNs leaving the site or organization. Uses those panels to train up new RNs and transfers the panel to the new RN upon successful completion of the orientation phase (first 90-days).

  • Develops and coordinates education experiences and clinical training for novice and newly hired staff on an individual and group basis and monitors progress of CCA Clinical Orientation Competency Program.

  • Develops individual training plans based on clinician's needs.

  • Provides leadership with respect to identification and dissemination of culturally and linguistically appropriate health education. Supports nurses safely practicing to the full extent of their education and training.

  • Participates in and uses evidence-based research to identify strategies for improving professional development activities, nursing practice and patient centered outcomes.

  • Ensures educational activities are congruent with CCA's mission, goals, values, priorities, and resources.

  • Assists adult learners to identify both their learning needs and the most effective activities to meet those needs.

  • Provides an appropriate climate for learning and facilitates the adult learning process by developing, planning, and presenting educational activities and directly/indirectly fostering the development of competence in the nursing staff.

  • With site leadership and HR support, completes 90-day performance evaluations of new RNs, LPNs and MAs and participates in performance management activities as necessary.

  • Upholds ethical principles in all aspects of practice.

  • Models behaviors that reflect affiliations with professional organizations as well as the community.

  • Support teams and track Key Performance Indicators (KPI) related to clinical care for members via telehealth technologies (video, chat, etc.) for a clinically appropriate clinical care and care management services.

  • Assists with the implementation of special programs related to clinical education

  • Manages a modified caseload as needed(don't want to be specific here)

  • Participates in the interviewing and selection process of novice clinicians

  • Responds when remedial help is needed for individual clinicians and helps site manager develop corrective action plans

  • Collaborates with others in the practice of nursing professional development at the institutional, local, regional and state level.

  • Works with the CCA Clinical Education & Staff Development Teams to standardize curriculums and training programs across the organization.
