Registered Nurse (RN) - Case Manager
Salary: $65,000 - $80,000 processed bi-weekly.
Shift Type: Full-time, Monday - Friday, 8 AM to 4 PM (flexible), on-call rotations.

Case management involves creating a Plan of Care for patients and following physician orders that lead to outcomes of increased patient independence and satisfaction. Responsibilities
- Assumes responsibility for assigned patient’s care from admission through discharge by effectively executing patient’s Plan of Care.
- Collaborates with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources.
- Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.
- Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.
- Participate in on-call rotation as reasonably assigned.
- Minimum of 2 years of nursing experience with a preference for 2+ years in an acute or a post-acute care setting.
- Active State Registered Nurse licensure is required.
- Proof of current CPR certification and valid Driver’s license.
