Evaluate the psychosocial needs of patients and their families/caregivers affected by terminal illness and their surroundings, sharing insights with the registered nurse and interdisciplinary team.
Maintain comprehensive clinical records for all social work referrals.
Conduct End of Benefit assessments for new admissions.
Offer information and referral services to patients, families/caregivers regarding practical and environmental requirements.
Provide pertinent information to patients, families/caregivers, and community agencies.
Act as a liaison between patients, families/caregivers, and community organizations.
Foster collaborative relationships with team members to enhance patient care.
Contribute to the development of personalized care plans and participate in interdisciplinary meetings regularly.
Assist physicians and team members in understanding significant social and emotional factors linked to health issues and end-of-life concerns.
Required Qualifications
Master's Degree in Social Work or Counseling Psychology.
Minimum of 1 year of experience as a Social Worker or Counselor.
Preferred Qualifications
Minimum of 1 year of experience in Hospice care.
Fluency in multiple languages.
Compensation
Per-Diem Rate: $65 - $80 per hour
