Position Details
Pay Rate: $45 per hour
Location: Metro Detroit Area
Qualifications
Minimum one year of RN experience in a Skilled Nursing environment
Current Registered Nurse (RN) license in good standing
Graduate of an accredited School of Nursing
Proficient in providing skilled nursing services in the home, including wound vac management, IV infusion administration, blood draws, wound care assessment and dressing changes, enteral nutrition setup and administration, medication administration, tracheostomy care, incontinence care, and comprehensive patient assessment
Ability to instruct and educate patients and caregivers as needed
Experience supervising LPNs and home health aides according to regulatory guidelines
Current and valid driver's license with reliable and insured transportation
Current CPR Certification
Current negative TB test or negative chest x-ray
Functioning electronic device with cellular service for field charting
Knowledge and experience with home health care electronic charting systems
Familiarity with Homecare Industry, Medicare/Medicaid/CMS/State of Michigan regulations
Essential Functions
Provide compassionate care to multiple patients in their home environments
Conduct thorough nursing assessments, including CMS OASIS documentation
Communicate significant findings to physicians and Clinical Manager
Implement physician orders accurately and in a timely manner
Ensure safety during treatment procedures and maintain a safe environment
Supervise and evaluate care provided by nurses and aides
Educate patients and family members on new diagnoses, care protocols, and support skills
Maintain accurate and timely documentation
Attend required in-services and meetings
Uphold HIPAA compliance and patient confidentiality
Position requires 100% travel to patients' homes
