Position Summary
Provides non-medical support to a caseload of older adult clients (55 plus) to enable clients to stay in their homes. The ideal candidate will provide light housekeeping, laundry services, shopping for food and or other household items as needed, light meal preparation, and running errands.
Conducts home visits and supports clients with light housekeeping, laundry services, shopping for food and other household items.
Provides companionship and personal engagement with appropriate and professional boundaries
Supports with light meal preparation and running errands.
Helps to remind clients of taking medications and/or remind them to keep record of medications
Helps clients get to and from medical appointments when needed.
Is part of the Older Adult Care Team and works closely with RN and Clinical Case Manager.
Collaborates and coordinates care with all primary and specialty care providers to arrange for care as dictated by the treatment plan.
Requirements:
3-5 years of working with older adults (55 plus); some knowledge of serious mental health illnesses preferred.
Beginner to intermediate experience with Microsoft Office products including Word and Outlook.
Experience in or willingness to learn how to use an Electronic Medical Record (EMR) system.

COMPENSATION RANGE :$17-$19 hourly
