Position Summary: The Care Coach is a nationally certified medical assistant who works directly with patients in one-on-one encounters and group sessions to reinforce self-management of their chronic condition while closely monitoring their established care plan goals.

Conduct patient face-to-face visits to promote established care plan goals and reinforce lifestyle modification education.
Collaborate with the Care Educator Nurse to discuss cases and ensure patient care gaps are closed.
Identify and see same-day patients who will benefit from the care education program.
Coach patient group workshops as organized by the Care Program Nurse Manager.
Accurately document patient information into the EMR system.
Participate in health fairs and health promotion events.
Perform other assigned duties by the Care Program Nurse Manager.
Required Education and Experience:
High school diploma or equivalent.
Graduation from a nationally accredited Medical Assistant (MA) program with active MA national certification.
Experience in Microsoft Word, Excel, PowerPoint.
Local travel to assist in covering other facilities within the same area.
Excellent oral and written communication skills with evidence-based data and etiquette.
Ability to think on their feet to solve problems for patients while achieving care program team goals.
Detail-oriented to ensure accuracy of reports and data.
Ability to maintain effective and organized schedules to ensure timely patient flow.
Ability to manage stressful and fast-paced work environments.
Certified Clinical Health Coach (certification required within the first 4 months of employment).
CPR/BLS for Healthcare Providers is required.
Preferred Education and Experience:
Two (2) years of Medical Assistant experience in a primary care setting.
Experience working with chronic disease education management.
