Position Overview:
Under the direct supervision of the Outreach Team Lead, the Community Health Worker is responsible for supporting Harris County Public Health’s outreach activities and serves as the key liaison to educate, promote, and connect community members to all HCPH programs/services specifically on behalf of the Environmental Public Health and Mosquito and Vector Control Division Programs. This position will primarily work in the community through various outreach activities within Harris County, while also completing in-office tasks/projects to support overall outreach team objectives.
HCPH Program Support: CDP and Dental
Duties and Responsibilities:
Responsible for a variety of outreach, education and community coordination activities including canvassing local businesses/organizations, attending community events, conducting outreach phone calls, and giving presentations.
Serves as subject-matter experts to meet outreach objectives for specific HCPH programs and recommends changes to improve outreach strategies. Also supports the creation/updating of HCPH program trainings for the entire outreach team.
Maintains knowledge/skills to educate, promote, and connect community members to all Harris County Public Health programs/services.
Tracks, collects, and submits outreach service data on a daily/weekly basis.
Builds and maintains relationships with stakeholders and organizations to create and sustain innovative partnerships.
Responsible for initiating referrals to HCPH programs/services, while ensuring community members' confidentiality.
Creates and delivers educational presentations/workshops for various public health topics in a for diverse target audiences.
Completes in-office tasks and projects for various team initiatives and grant deliverables
Responsible for establishing a strong working relationship with managers and all employees

Requirements
Education:
High School diploma or GED equivalency
Knowledge, Skills and Abilities:
Intermediate proficiency in Microsoft Office

This job operates in a professional office environment.
This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
Physical Demands:
This is a largely sedentary role; however, some duties would require the ability to lift up to 20 pounds and bending & standing for periods at a time.
