Hours: 8:00 - 5:00 Monday - Friday, 40 hours per week (some evenings and weekends)
The Community Health Worker (CHW) is a grant-funded role. The CHW will act as a patient advocate or liaison in a community setting. Building trust, promoting encouragement, and providing social support are the key objectives of this role. The CHW helps patients access community services specific to their needs. The CHW assists in creating community outreach programs and activities. These outreach programs are designed to promote, maintain, and improve the health of the community. This is an integrated, grant-funded role that will partner with various organizations.
Primary Responsibilities
Provide health information to community members and encourage healthy living habits
Provide COVID-19 vaccine information among priority populations
Assist patients in referrals, follow-ups and completing applications for services
Conduct intensive outreach efforts to community members and agencies serving vulnerable, high-risk populations
Advocate for individuals and communities within the health and social service systems
Education/Experience: High School or equivalent. 1-2 years of experience in community outreach within a multicultural setting. Written and oral fluency in English and Spanish preferred.
License: Valid NC driver's license
Knowledge, Skills & Abilities
Ability to multi-task, prioritize and organize work while meeting deadlines
Must be flexible and able to adapt to a changing work environment
Ability to effectively communicate with a wide range of individuals and among a diverse population
Must be proficient in medical terminology
Knowledge of basic computer programs and Microsoft Office software
