Case Planner

PRINCIPAL DUTIES AND RESPONSIBILITIES:

• Completes all in-person home and school visits as per requirements
• Completes the required web-based training.
• Completes the required in person training and passes the follow-up exam.
• Meets new families to complete the CASI and then the Comprehensive Family Assessment (CFA) using Evidence Based tools.
• Works with families to select the relevant Core Outcomes from the CASI to set up SMART GOALS with the families.
• Creates the family FC Outcome Driven Service Plan and the ACS FASP for the families.
• Demonstrates effective use of Change Focused Interventions developed in the Service Plan to help families to achieve their goals.
• Implements Service Plans by coordinating, collaborating and /or providing preventive, family counseling, and educational services in the family home and community.
• Provides ongoing Safety and Risk Assessment throughout the FC process to ensure child safety.
• Participates in FTC as per ACS guidelines.
• Works with families and Program Supervisors to complete services within the 12 month ACS guideline.
• Documents client’s progress notes and Family Assessment Service Plans (FASP) in the NYS Data base system (CONNECTIONS) within required timeframes.
• Documents frequency of contacts with clients in the ACS data base system Preventive Organization Management Information System (PROMIS).
• Links clients to information/resources to services that will support them in the community.
• Completes Workforce training requirement and other in-service trainings to improve skills to effectively meet the needs of referred families.
• Actively participates in Supervision both group and individual
