Join Nystrom & Associates’ Behavioral Health Home (BHH) team as an intern and gain hands-on experience in providing case management services to individuals with mental health diagnoses. You'll be part of a community-based team making a major impact by connecting individuals to essential mental health services, preventative care, and community programs.
About Behavioral Health Home (BHH)
BHH is a lower level of care and a step-down from services like Adult Rehabilitative Mental Health Services (ARMHS), Targeted Case Management, or Assertive Community Treatment (ACT). The BHH model is team-based, including a case manager, nurse, and a community health worker.

Internship Responsibilities
Client Connection: Assist in connecting clients to mental health services, preventative care, and community programs such as housing, food shelves, and employment.
Care Transitions: Support clients during transitions in care settings.
Holistic Care: Provide culturally responsive, holistic care within the community.
Team Collaboration: Work closely with nurses and case managers to deliver comprehensive care.
Health Promotion: Help clients develop skills related to overall health and wellness.
Client Interaction: Engage with clients monthly through phone or in-person meetings.
Qualifications
To be eligible for the internship, you must meet one of the following:
Be enrolled in a Psychology or Social Work program and be a Junior or Senior.
Have a valid driver’s license, auto insurance, reliable vehicle, and access to the internet.
Live near the Apple Valley, Duluth, Cambridge, or Maple Grove clinic locations. The internship could be based out of any of these sites, depending on the intern's preference.
Be able to work remotely, having a private space that cannot be overheard by others.
Be able to work independently and travel within a 30-mile radius of the primary clinic location. This internship will be primarily remote, with potential travel to clients depending on client needs.
Commit to a minimum six-month internship, with a preference for those able to commit to nine months.
Preferred Qualifications
Previous experience working in settings such as: emergency services, in-home/group home services such as ILS or PCA provider, group therapy services such as Behavioral Specialist roles.
Experience working with diverse communities.
Interest in working with clients with mental health diagnoses.
Knowledge of community resources and health interventions.
Desire to work with children, families, and adults in their homes, including physical activities such as walking, climbing stairs, crouching, stooping, sitting on floors, and standing for extended periods.
Proficiency in using computers, including Microsoft Word, Excel, Outlook, and Electronic Health Records (EHR).

Flexible Schedule: Create your schedule within business hours, Monday-Friday. Part-time - shifts must be worked in four-hour blocks and interns must commit to a schedule of 20 hours a week at minimum.
