But this doesn't resolve the issue that there is excessive and unnecessary documentation mandated of clinician. what happened to the SOAP notes that was clinically relevant. why all the extra fluff expected of physicians to document? Hospitals nationwide are grappling with capacity constraints, leading to prolonged patient stays and escalating costs. A recent analysis by Kaufman Hall highlighted that reducing length of stay is closely linked to stronger financial performance, as patients move through the system more efficiently. According to the latest "National Hospital Flash Report" for April 2024, the average length of stay has dropped by 5% since 2021, although it has remained relatively flat year-over-year. Prolonged patient stays can stem from various factors, including inefficient discharge processes, staff shortages, communication breakdowns, and a lack of post-acute care facilities for patients who cannot be discharged home. As healthcare leaders, we must prioritize strategies to streamline patient flow, optimize resource utilization, and enhance care coordination. I truly believe that virtual/ AI based scribes will finally create the much sought for, true acceptance of the EHR era for physicians and other professionals! I will highly recommend that clinicians and their administrators should start some level of planning and experimentation of early pilot programs with various AI-based scribing options. Like any innovative solution there will be economical, work flow and regulatory challenges, but nothing I believe will be insurmountable. Good days are coming-- days that will finally put an end to the disruptions that were initiated by the paper to digital transition! The coming generations of clinicians truly owe a lot to the previous and our generations, who suffered through the tough paper to digital transition, and paved the way for a better future, both for clinicians and the patients! Amazon's effect on physician consolidation Becker's Physician Leadership Review published this in early April- As physicians migrate to employed models and the healthcare industry consolidates, retail giant Amazon continues to double down on its focus on primary care and other physician specialties.  Amazon has 221 primary care offices in more than 20 markets. In February 2023, it finalized a $3.9 billion acquisition of virtual and in-person primary care company One Medical, which added more than 200 brick-and-mortar physicians offices and roughly 815,000 One Medical members. The One Medical deal has already "changed the landscape" of healthcare and altered the way hospitals do business, health system leaders told Becker's. "The acquisition of One Medical by Amazon should remove any lingering doubt that Amazon is serious about making a real and substantive move into healthcare as well as cementing the notion that any viable healthcare strategy must have at its foundation a robust primary care offering," Richard Zane, MD, chief innovation officer for Aurora, Colo.-based UCHealth, told Becker's.  Amazon has since launched One Medical for Prime, offering Amazon Prime members unlimited virtual visits for $9 a month. In January, Amazon Pharmacy integrated with One Medical to give patients and providers increased access to medication consultations. One Medical provides concierge primary care, with longer appointments and no wait times, as well as 24/7 virtual care for a yearly membership of $199 and has formed collaborations with 19 hospitals. The hybrid primary care company has specialty care referral agreements in the health systems' local markets.  The deals allow specialists from health systems to deliver care to Amazon's primary care patients.  Some of those health systems told Becker's in September the partnership improves access to primary care in their markets.  "This partnership with One Medical is a prime example of Mass General Brigham's commitment to working with world-class partners that complement our strengths," said Lynn Stofer, president of Mass General Brigham Community Physicians, part of the Somerville, Mass.-based health system. Keep our rurals open but if you really want to help get the foreign equity owned GPO that sets the pricing for our hospitals BANNED from our state. Economies of scale are used by Vizient against Oklahoma rural hospitals. If you have 13 tiers, hidden tiers, and tier maxes…that’s not a contract, that a free for all nightmare of a scam we all fell for. Their awful written tiered agreements force our rurals to pay more for the exact same items that our large hospitals pay less for. Example box of gloves a large hospital system in the state will pay 6-8 bucks for a box of 250 exam gloves. That same box for a rural EMT? 24 bucks. There is no price transparency and you are forcing patients who live in rural areas to pay more because of the lack of infrastructure by the corporations serving them to effectively and economically get goods and services to these hospitals. And distributors who whine about the cost of delivering to rural hospitals all while posting BILLIONS in profit every quarter are the first on my 💩 list. Let’s keep our rurals open but let’s make them more profitable so that they can continue to add much needed services and gainfully employ more community members.

Volumes: Implement software solutions to efficiently handle pt. volumes. OP CDI Calculator: Utilize a calculator to determine Full-Time Equivalent (FTE) needs. Data Analytics and Reporting: Emphasize the importance of continuous improvement via data analytics. 3. Workflow Creation Establish Robust Workflows: Create strong workflows before integrating reviewers. 4. For instance, a CDI director I guided successfully identified HCC and HEDIS recapture as their biggest pain points. With strategic solutions in place, they are now ready to implement the project for the 2025 budget yr., aiming to improve their HCC recapture rate by 20%, leveraging data analytics for outcomes and improvement needs. Benefits of Implementing These Strategies Align Risk Stratification: Properly align risk stratification with chronic conditions and healthcare needs. Quality Measures: Improve quality measures and address SDOH. Collaborative Team: Ensure success with a collaborative team supported by technology.
