In a recent study that included individuals at elevated risk for Alzheimer's disease, researchers observed that those who did not develop the neurodegenerative condition carried a gene variant that helps the brain more efficiently remove the beta amyloid plaques from the brain that are a hallmark of the disease. If scientists are able to develop a therapy that mimics the gene’s effect in the brain, it’s hoped that Alzheimer's cases may be reduced by up to 70%. Columbia University, April 2024 Diet: Intermittent Fasting Benefits Intestinal Health. Intermittent fasting is a dietary strategy that alternates between periods of eating and fasting, typically within a structured time frame each day or week. A recent animal study found that intermittent fasting is associated with reduced inflammation and improved glucose control in the small intestine, which suggests this dietary practice may improve intestinal health. American Physiology Summit, April 2024 Exercise: Sedentary Behaviors Raises COPD Risk. Chronic obstructive pulmonary disease (COPD) is a chronic lung disease that makes breathing difficult, and it is a leading cause of death in the United States. An analysis of data from several long-term studies found that for every 2.8 hours an individual averages per day watching television, their risk for COPD doubles. BMJ Open Respiratory Research, April 2024 Chiropractic: Manual Therapy's Impact on Ankle Dorsiflexion. For individuals with reduced ankle dorsiflexion following a lower limb injury, a recent study found that ankle joint mobilization and calf muscle massage are effective interventions for improving ankle range of motion. Doctors of chiropractic are trained in the use of these and other therapies in the management of musculoskeletal disorders of the extremities, including the ankle and foot. Journal of Bodyworks and Movement Therapies, January 2024 Mental Attitude: Long Commute and Musculoskeletal Pain Linked to Burnout. Questionnaire completed by 1,615 healthcare professionals revealed that the combination of a commute lasting longer than 50 minutes and either neck and shoulder pain or ankle pain alone are linked to an elevated risk for burnout. BMC Health Services Research, April 2024 Wellness/Prevention: New Urine Test May Streamline Bladder Cancer Follow-Up. Researchers have developed a new urine test that can look for genetic markers of bladder cancer, which may reduce the need for follow-up cystoscopies in bladder cancer survivors while also detecting cancer recurrence for earlier treatment. Between 2021 and 2022, poison control centers experienced an 88% increase in calls related to synthetic cannabis products, particularly involving accidental exposure to young children, with one-in-four requiring hospitalization. Clinical Toxicology, April 2024 Diet: Are Ultra-Processed Food Linked to Premature Death? Ultra-processed foods are food products that include added sugars, fats, and salt to enhance flavor and prolong shelf life. Using data from two long-term health studies, researchers report that a high intake of ultra-processed foods is associated with roughly a 4-9% elevated risk of early death. BMJ, May 2024 Exercise: Exercise and Brain Function. In a recent animal study, researchers observed a new mechanism by which exercise benefits brain health. The research team observed that exercise alters gene expression in the microglia in a manner that appears to reverse the effects of aging, resulting in better immune function in the central nervous system, including the brain. Aging Cell, May 2024 Chiropractic: Shoulder-Specific Exercises Can Help with Chronic Shoulder Pain. An analysis of data from 54 studies found that exercise therapy is an effective first-line treatment for patients with chronic shoulder pain. Doctors of chiropractic routinely use a multimodal approach that includes therapeutic exercises when managing patients with shoulder pain. PLoS One, April 2024 Mental Attitude: Social Online Training Can Help Against Loneliness and Depression. For individuals who are unable to engage in regular face-to-face communication due to illness or disability, researchers have found that participating in an online social training program that includes daily interactions with another person focused on asking questions and listening empathetically is effective for improving resilience, empathy, and social connections while reducing loneliness, depression, and anxiety. Journal of Affective Disorders, March 2024 Wellness/Prevention: Oral Rinse May Spot Stomach Cancer Early-On. Researchers from the Rutgers Robert Wood Johnson School of Medicine report that saliva samples collected from individuals with stomach cancer or pre-cancerous stomach conditions exhibited differences in bacteria from healthy individuals. The finding suggests that an oral rinse test could be developed to screen for stomach cancer as part of a routine wellness visit. Digestive Disease Week, May 2024

Recent studies have found that about 16% of kids and teens have high blood pressure, and hypertension is becoming increasingly common in this age group. Among a group of more than 150,000 individuals whom researchers monitored from childhood to young adulthood, those with hypertension at the start of the study were two times more likely to suffer a major adverse cardiac event in the following 13 years. JAMA Pediatrics, May 2024 Diet: Follow Healthy Eating Social Media Accounts. According to a recent study that included 52 young adults, those asked to follow healthy eating social media accounts increased their produce intake by 1.4 servings a day and reduced their processed food consumption. Meanwhile, participants asked to follow interior design social media accounts experienced no such changes in their eating habits. Digital Health, April 2024 Exercise: Staying Fit Protects Kids' Mental Health. According to a recent study, higher performance in cardiovascular activities, strength, and muscular endurance is linked to a reduced risk for symptoms associated with depression, anxiety, and attention-deficit/hyperactivity disorder in children and teens. JAMA Pediatrics, April 2024 Chiropractic: Improving Scapular Function Helps Reduce Neck Pain Intensity. A systematic review that included data from eight randomized controlled trials concluded that treatment to improve shoulder blade function can reduce pain intensity in patients with chronic neck pain. BMC Musculoskeletal Disorders, April 2024 Mental Attitude: Most Parents Are Lonely. A national survey conducted by Ohio State University's Wexner Medical Center revealed that two-thirds of parents feel isolated, lonely, burned out, and unsupported, and these feelings are even more common in those who work remotely and have fewer personal interactions with their associates. Ohio State University, April 2024 Wellness/Prevention: AI May Spot Rare Disease in Patients Years Earlier. Researchers from the University of California, Los Angeles have developed an artificial intelligence (AI) agent capable of reviewing patient health histories to identify those at increased risk for a rare immune disorder called common variable immunodeficiency. It’s hoped this technology can be refined and used to diagnose the disease sooner in its course when a satisfactory outcome may be more likely. Science Translational Medicine, May 2024

Medicare is seen as the cornerstone of health insurances in this country. However, continuing cuts and cumbersome regulations related to reimbursements have caused some to think about opting out of Medicare. When a physician opts out of Medicare, it's not intended to be a knock on seniors, rather it's them trying to gain back some of their autonomy that has been eroded over the years, and they are tired of jumping through all of Medicare's hoops. So what does it mean, and how do you actually opt out of Medicare? I've been opted out for about 4 years, and this is what I know. More explained in the Insurance Free Medical Practice Course. 1. You can opt out with prescriber privileges. This is what I have done, and it allows me to still prescribe things for my patients that have Medicare, even if I don't take Medicare 2. Not everyone can opt out of Medicare: Physicians can, but physical therapists and chiropractors often cannot 3. To actually opt out you need to sign an affidavit with CMS which is handled through third party contractors. I think I sent an actual letter. Once you opt out, you can't opt back in for another two years, and if you don't do anything, it will continue opting you out every two years. 4. The only way you can legally charge a Medicare patient cash for covered services is by opting out, and having them sign an extra form that informs them that you have indeed opted out of Medicare, and they acknowledge this. 5. With private health insurances, you can give the patient a superbill and they can submit for out of network benefits. You can't do this with Medicare. I've been opted out for 4 years and have never looked back. 99% of providers still take Medicare, so there are still plenty of places for those with Medicare to go, despite my absence! #Medicare #healthcare #medicine

Basing your whole healthcare practice on only seeing patients with health insurance seems like a safe bet. It unlocks giant pools of patients for you to see. Seeing a lot of patients means that you have a successful practice, right? Wrong. That used to be the case when health insurance companies didn't require an army of staff for revenue cycle management, and they paid more. A few decades ago you could open your own practice, take insurance, it would get filled, and it would be financially stable. Now, only taking patients with health insurance will certainly make you busy, but it will also make you fail. If not now, then eventually. There are two reasons for this: 1. You can't control your overhead, and it always goes up. Controlling overhead is necessary for any business. Health insurance companies make you hire an army of staff to successfully bill. The HR costs alone are daunting and are only going up. 2. You can't control what you get paid. Another aspect you need for business solvency is to be able to set your prices. Health insurance companies dictate what you get paid, and it of course goes down over time as well. The combination of not controlling your overhead or your prices will lead even the best run practice to ruin. A health insurance based practice will be a busy practice that will eventually fail. The solution is to start looking elsewhere for payments.

If you had to guess, how much do you think it costs for a doctor to file a fully compliant professional incorporation for their private practice? $100, $500, $5k, $50k, $100k?, more? 🤔 As much as I would love to give you an answer, there is no set number because incorporations for private practices are still a manual process completely managed by legal professionals and CPAs, who usually pick and choose how much to charge without any consistency in pricing. Now, I am all for professionals charging for their time appropriately. However, I am 100% against the lack of automation and price transparency that prevents millions of doctors from creating their own businesses. Because the complexities of incorporation and the costs associated with them are an example of the administrative bloat of the traditional private practice model that can be completely eliminated with AI. That's why Ease is automating the entire process of starting a private practice, making professional incorporations instantaneous. But understanding and interpreting various state licensing and licensing board regulations is the limiting factor here. Upon training a data model with this information, the drafting and filing process, which typically costs $10k to $100k+ in legal fees and can take weeks, if not months, will be reduced to minutes or seconds. 🤯 You're probably thinking, well why hasn't the EHR solved this, well in spite of the fact that practice management software has existed for years, doctors can only use this software AFTER they have formed their practice. Hence, if we want to INCREASE the supply of new practices and INCREASE access to care to encourage more competition, we must automate the entire incorporation process, which is a critical first step in getting a practice built. Why your medical team matters.    Family offices partner up with wealth and legal advisors to assemble a dream team for their clients. I strongly believe it is just as important to make sure your medical team is optimized and communicating to mitigate health risk.   _ _ _ 3 reasons to put medical teams on your radar.    A connected team means access to more expertise.  When a cross-disciplinary team works well together, their collective intelligence leads to better solutions. A cohesive team means care is not delayed. Look at it this way: a geriatrician should have a hearing specialist on staff, a mental health program should have a neuropsychologist on staff, and a nursing home should have a podiatrist on staff.    And nothing falls through the cracks.
