Upward Health is an in-home medical group that specializes in primary medical, behavioral, and social healthcare for individuals with complex needs. We deliver care in the community. We reduce barriers to care, such as long delays due to scheduling or lengthy commutes to a provider’s office by meeting the patient right at home. We see patients when they need us, for as much time as they need. Beyond medical and behavioral care, we also assist our patients with challenges that may affect their health, such as food insecurity, social isolation, housing needs, transportation and more.

Upward Health is working with Aetna, a CVS Health Company, in Arizona to launch our whole-person model of care to a cohort of Aetna Medicare Advantage members who have multiple, chronic health conditions. Our team works seamlessly with members’ existing provider teams, or directly with members without an established provider, to improve coordination and access to care primarily through a home-based care setting. The goal of the collaboration is to improve the health and well-being of this Medicare population with a focus on delivering preventive care in the community to keep members healthy. Improving access to care and offering high-quality care is what we do best.
