We are a skilled nursing facility located in the heart of Thornton, CO. At almost 250 beds, we are proudly the largest nursing facility in the state of CO. We provide both short-term rehabilitation stays and long term care. We have a secured memory care unit and offer both private & semi-private rooms on both secure and non-secure units. With an oversized therapy room, multiple dining areas, activity rooms and our community of people - you will have everything you need to feel right at home whether you're here short-term or long-term.

    Skylake Post Acute, a PACS supported nursing facility in Colorado, has a long-term resident who’s been a Denver Broncos season ticket holder for years. He met another long-term resident who's a lifelong New York Jets fan — but had never been to a game. “This patient took it upon himself to offer his tickets to the resident, who's a Jets fan, for the next home game," said Grady Goldhammer, Skylake administrator. "I later found out that our Director of Nursing also took time out of his dayoff to take our residents to the game!" What a life-changing experience for our everyone involved. It truly shows the type of people that work in PACS supported facilities around the country. Bravo Grady and Team!! #caring #teamwork #peoplefirst
