"In care management, we work hand in hand with patients who need cancer screenings. My job is to help them make sure they complete their work up and overcome any barriers, such as authorizations, transportation, translations, etc. Even though my work is done over the phone, I make sure the patient understands and is agreeable to what's next. Through one-on-one education with patients, I want to help them feel confident and reassured throughout the process.    "I had one patient who was just not sure about getting the colonoscopy done and procrastinating it, but with some encouragement and education, I was able to help the patient get the colonoscopy done. They found a pre-cancerous polyp. But luckily, the polyp was removed well in time and the patient was glad our persistent encouragement helped them along the way." Meet Sherry: Sherry, a TrueCare patient, sought assistance to set up her MyChart account. What ensued was a remarkable digital literacy transformation. With the help of Yolanda, a TrueCare MyChart Support Specialist, Sherry not only created her account but also learned how to use a tablet she received through the OPSA program at TrueCare San Marcos.   Yolanda showed Sherry how to navigate the tablet and the MyChart app. Over several meetings, Sherry mastered sending medication refill requests, scheduling appointments, and viewing lab results. She’s even learned to read text messages, so she has the option to book telehealth video appointments when needed.
