AccessHealth Community Health Center

We have great news to share. We’ve received a 2024 Houston Methodist DEI grant. The DEI grant helps AccessHealth take care of our diverse patients, most of whom are from different racial or ethnic backgrounds, by supporting two Community Health Workers (CHWs). These CHWs help make our neighborhoods healthier by providing healthcare services and improving the quality of life. They also train AmeriCorp members to use our Health Navigation Platform (HNP), which connects patients with local resources to meet their basic needs. Now in its fourth year, the DEI Grant Program will support more than 52,000 people in the #HTX community through 22 healthy neighborhood programs, 12 economic empowerment programs and 13 educational empowerment programs.

How in the world can attorneys and lawyers improve health?? When they're part of important medical-legal partnerships stationed inside clinics working to help Texans facing the most barriers to good health! AccessHealth Community Health Center just created Fort Bend County's first legal outreach program. Funded in part by EHF, the program screens the non-medical needs of patients and then deploys attorneys that can help with issues like safe housing, protective orders, and other legal matters that may impact their health. “All of these things are run through our care coordination team, and they connect patients with the resources they need to live a quality, healthy life,” says Danish Nelson, AccessHealth's senior brand and marketing manager. Last Month, AccessHealth Community Health Center Palak Jalan WIC Director, Priscilla Ayanwale Cole and staff, Fort Bend County Precinct 2 Grady Prestage and Brenda Patton and staff, Fort Bend County Parks and Recreation Darren McCarthy and staff, Fort Bend County HHS’ Community Health and Wellness Division Manager Gwen Sims and Staff, OCEE Community Health Engagement Coordinator Barbara Irving and staff, YMCA of Greater Houston Jennifer Pinti and Houston Food Bank collaborateed to launch the open house for Women, Infants and Children (WIC) services at the Mustang Community Center! WIC is for pregnant, postpartum and breastfeeding women and families with children up to age 5. Empowering women through action! 🌟 AccessHealth is thrilled to kick off the 'Wherever Women Are Project' with Organon's 'Her Plan is Her Power.' We're honored to be part of Organon's U.S. grant programs, focusing on reducing unplanned pregnancies in high-need communities. This includes funding for community health clinics in Texas, Ohio, and Georgia, along with support from Direct Relief’s clinic network. AccessHealth was the starting point for Organon's listening tour, engaging local stakeholders and healthcare teams to identify and address barriers to reproductive care.
