Happy to be back in Hong Kong for the 醫院管理局研討大會. I look forward to speaking at the Hong Kong Health Authority 2024 global gathering. Traditionally, health in Hong Kong has offered high-value care for just 7.4% of GDP but, like other countries, is now experiencing a significant workforce shortage as the population ages and waiting lists grow. Hong Kong is facing a serious shortage of doctors, where its per capita medic ratio lags behind other advanced economies. Currently, Hong Kong has a ratio of 2 doctors per 1,000 population, far below the ratio in Singapore (2.5), Japan (2.5), the United States (2.6), the United Kingdom (3.0) and Australia (3.8). Historically, Hong Kong has operated a tight, rather insular and protectionist, medical registration policy which it’s now liberalising at a rapid rate. Most of the doctors attracted by this policy are from the UK diaspora, where the pay difference is significant. The salary of a doctor of equivalent standing in Hong Kong is roughly double that in the UK. Hong Kong looks to Singapore where 30% of its medical workforce come from overseas. While these rates will not be matched in the SAR, it’s clear there is ample room for growth. And, like other Asian countries, investment is being made in infrastructure to push virtual care, cloud, associated technologies, Ai and robotics. This is an interesting policy idea and I personally believe we must share data to save lives. Faced with a choice between data privacy and health what would you choose? Central public ownership is one model but what about alternatives like Financial Services which has privately owned but heavily regulated credit bureaus? It also has open banking these days….. What’s your view on which would deliver the most value and be acceptable to the public?

One of the many shapes of the things to come?  It is often (or even always) the unintentional consequences of new technology that captures the wider imagination and then often lends itself to private equity backed enterprise rather than its potential being realised in the slow moving, organic, but robust world of medical and scientific study and rigour. These are examples of the ‘non-sharp’ sidedness of the Occam Razor phenomenon (which we have wrestled with, but arguably poorly applied over the centuries). Many organisations now focused on CRSD and in business readiness preparation. Department of Enterprise, Trade and Employment has good detail on definitions and timing. The Corporate Sustainability Reporting Directive (CSRD) arises from the European Green Deal’s climate change action objectives, to further enhance the disclosure by companies on climate and environmental data. CSRD scope and timeline Ireland and other member states have until mid-2024 to transpose the directive, with a view to mandatory requirements commencing for financial years on or after: 1 January 2024 for public interest entities in scope of EU non-financial reporting rules (greater than 500 employees) 1 January 2025 for other larger companies and public interest entities (greater than 250 employees) 1 January 2026 for listed SMEs, with an ‘opt out’ possible until 2028 The Dept website has more details. With nearly 25 years of experience in the healthcare sector, and a passion for improving people’s lives through healthcare transformation even before my hospital care days, I’m proud to have co-authored important joint research from Boston Consulting Group (BCG) and the NHS Confederation on addressing the growing issue of long-term sickness and economic inactivity. In the UK, we have 750,000 more people who are economically inactive due to long term sickness since the pandemic. This is in stark contrast to our EU peers, who have seen a 2.3% fall in economic inactivity, while ours has risen by 1.1%. Disturbingly, those aged 18-24 are among the most affected, averaging 18% annual growth rate between 2019 and 2023 for those reporting five health conditions. This issue not only impacts individual wellbeing, creating a significant social need for action, but also poses serious economic and fiscal costs. Addressing this problem can transform the health and lives of hundreds of thousands of people and boost UK economic growth by £109-177bn in the next five years. Over 50% of health outcomes are influenced by non-healthcare factors related to the places they live, such as living conditions, crime rates and access to services. A comprehensive whole-of-government approach (WGA) is therefore critical to tackle these root causes. Second NHS Confederation Expo session of the day, hosted by Microsoft! An AI driven future: enhancing clinical productivity 💻 Great to hear Henry Morriss open the conversation with a recognition that there is a lot of exciting potential to utilise AI in the healthcare setting. However, we need to look at some of those core challenges that still exist - such as the laptop taking 20 minutes to turn on! Ensure that a 'stable environment' exists in the foundations of the technology is imperative. 💙 Jacob Lant's point around personalised care particularly resonated regarding 'removing the screen' during consultations. Through voice recognition tools, we can properly engage during interactions with patients, capturing both the clinical facts but also those wider determinants of health. Overall, providing that more 'personalised care' approach. 📄 Some great example from Anna Dijkstra on how this AI can be applied across a variety of different use cases and setting, including the use of AI to scan hundreds of policy documents across a Trust's HR department to answer specific questions for staff. 🤝 Addressing digital and social deprivation - as called out by Jacob, with c.8.5 million citizens without the skills to engage with digital, and c.2 million without broadband due to affordability, we must ensure that as the technology continues to progress that we are mindful of exclusion.
