1$\begingroup$ You are using $t$ to mean two different things. In the line $v = s/t = ...$ you are using $t$ to mean the time interval from start to finish of the motion, i.e. the value of time at the end minus the value of time at the start. When you then do $ds/dt$, you are using $t$ to mean the time value at the end of the motion.
