The treatment landscape for geographic atrophy has changed dramatically. For the first time, we have treatment options with the FDA approval of 2 drugs: pegcetacoplan injection and avacincaptad pegol intravitreal injection. In light of these advances, there is renewed interest in following patients with AMD using multimodal imaging and optical coherence tomography to find the early signs of GA. She had a well-centered PCIOL and on fundoscopy, an intraretinal hemorrhage was noted. The 𝗦𝗣𝗘𝗖𝗧𝗥𝗔𝗟𝗜𝗦 𝗠𝘂𝗹𝘁𝗶𝗖𝗼𝗹𝗼𝗿 (MColor) image shows non-central geographic atrophy (encircled in red) and drusenoid changes (encircled in white) with dark-green coloration centrally (deep retinal fluid). 𝗜𝗥 image shows light-absorbing material in the macula (pink circle). 𝗢𝗖𝗧 shows IRF and intraretinal fibrovascular tissue (type 3 MNV) with RPE and photoreceptor alterations. 𝗜𝗖𝗚𝗔 (2m50s) shows a window defect (encircled in red), and absence of distinct abnormal choroidal vessels at the inner retina leakage site close to the fovea on FA, confirming type 3 MNV (grey circle). During the video, the image contrast and brightness can be enhanced to highlight details with fainter signal (red framed). Therefore, keep the “Alt“ and the left mouse key pressed, and then move the mouse to the left or right to modify the contrast (window width), or up and down to modify the brightness (window level).
