uhid: Add host and peer address to uhid attributes
Match what bluez does when creating a uhid device and set the host
address as attr{phys} and the peer address as attr{uniq}.

TEST=cros deploy to eve, connect LE keyboard and verify with
udevadm info -a -p /sys/devices/virtual/misc/uhid/.../input/...
